The Netherlands won Group G with 20 points, while Poland, who finished second with 17 points, will appear in the play-offs.
The Poles defeated Malta 3-2 on Monday.
For Poland, the goals were scored by Robert Lewandowski, Pawel Wszolek and Piotr Zielinski.
Irvin Cardona and Teddy Teuma scored for Malta.
Finland came third in the group with ten points, while Malta came in fourth with five points.
Lithuania last with three points.
